Introducing LifeBridgeThe LifeBridge is the latest innovation at Intercept intended to address the growing and changing needs of the child welfare system in Virginia. As a system of care, it represents both a philosophy and a process by which to swiftly and successfuly re-integrate youth into the community with permancy ties. Philosophically, the treatment and milueu are designed to accomplish the most efficient and succesful discharge. As a system, The LifeBridge is a continuum of community based services designed to cover the full spectrum of needs for our youth, and, most importantly, to coordinate the movement of youth through these services until successful re-integration into the community is achieved.
At the heart of the LifeBridge is the "Community Re-integration Plan", a comprehensive plan tailored to the individual and directed toward the individual's successful re-integration into the community, whether that be through the rejoining of the individual's natural family, a foster/adoptive family, or is through a successful transition to independent living while maiantaining permanancy supports. Intercept works with the individual client as well as assesses the natural community supports, adapting both to each other, in order to ensure the most efficient and successful discharge. The Community Re-Integration Plan is a living document, continuously updated based on the progress of both the individual and the community support mechanisms. Overseeing the Community Re-Integration Plan is a LifeBridge Care Coordinator, who ensures that the plan considers the needs of the youth, the youth’s family, and the caseworker and that it helps the youth navigate through the continuum of necessary services.
The LifeBridge is a true continuum in that it provides a seamless transition between services. Well in advance of discharge, the relationship between the youth, their family and the intended counselor is established thereby providing familiarity and comfortability in an ordinarily stressful discharge situation. The result is a smoother transition with less potential for recidivism. Whether used individually or as a continuum, Intercept’s array of services results in efficient and successful community placement and are widely recognized as among the best in Virginia.
