Mental Health Support Provides instruction and counseling to individuals who, due to mental illness, would otherwise have trouble maintaining community stability and independence. Focus areas include daily living, community resources, health and safety, financial management and social supports. Services are individualized, and provided both within the client’s home and their community.
Transitional Services A precursor to Intensive In-Home and Mental Health Support, Transitional Services allow clients in congregate care and Independent Living settings to seamlessly reintegrate into the community, proactively, by both establishing services and building a rapport with the client and family prior to discharge.
